OUR SERVICES

Our carers are available for day shifts offering as and when required; this could be morning, lunch, tea time or bed. Day care may be desirable for someone who needs assistance with personal care such as showering, dressing and at meal times, but does not require around-the-clock care. Having a part time carer can greatly improve the quality of life for your loved one as well as everyone else involved.

PERSONAL CARE & HYGIENE

Support for outing and outdoor activities which include shopping or helping to make a shopping list and go to the park.

SOCIAL CARE

This include home help, carrying out tasks such as washing, cooking, and cleaning and any household chores that need completing. We offer full assistance with all aspects of personal care: bathing, showering, hair washing, dressing, oral health, shaving and toileting. All care will be carried out with dignity and respect and in a person-centred way. The carer will be ale to assist with moving and handling tasks and other aspects of mobility, as well as assisting with safe administration of medication an dietary needs.

HOME CARE

Mealtime support such as preparing meals with or for you and assisting you at mealtimes.

MEALTIME SUPPORT

Support with medication which includes reminding and assistance or collecting and returning medicines from your pharmacy or dispensing GP surgery.

SUPPORT WITH MEDICATION

This includes support for social activities like going out for a walk, attending a day centre, visiting friends or family or going to your church or club.

ACTIVITIES & COMPANIONSHIP

Help with mobility and movement such as getting up, moving up and down stairs, reaching objects and other such activities. We tailor each service to consider the individual requirements of each client, considering all aspects, including specific needs, daily routines, and personal preferences.

At Swan Care we see Dementia as symptoms that describe a condition. We understand feelings of losing your loved one but it doesn't have to be this way. We are in this journey together with you and for you that promotes your health and well being.

We will be there in your own home that is familiar to you and provide the support to you for you being able to stay in the familiarity of you own home with one-to-one, tailored support can all make a real difference.
